I need it in Excel, add a toolbar that opens a form when you click on it, using only VBA code?

I an Excel document. When it opened, add a custom button to the toolbar that opens a form when clicked. I have the form ready and I understood how to add the button by setting "Application.CommandBars (" Standard "). Controls.Add: = msoControlButton Type" in Private Sub Workbook_Open (). But I can not understand how to change anything on the button. It's just a blank button that will do nothing. I did not know how changed manually right-clicking/Customize/Modify Selection / Assign a macro, etc. But it will not work. It has to all be automated in VBA code.
