outlook forms publish

I am making a form in Outlook (windows xp, office 2000) and cannot add attachments to it, why?
If I publish the form and try to send with it, the insert file button is greyed out and I can’t paste or drag and drop into any of the text fields. Is there a setting I need to change to allow this?
Do the form on Word as a document , save it and then attach the file.
